Job summary

Millennium Home Care, LLC is seeking a passionate PRN Physical Therapist in Florida to provide in-home care with flexible scheduling and competitive pay.

You will conduct patient assessments, design and implement treatment plans, and guide families on exercises and safety. Collaborate with physicians and nurses to ensure optimal outcomes and maintain accurate documentation in a home health setting.

Qualifications

• Current and valid Physical Therapist license in Florida.
• Minimum of 1 year of clinical PT experience, home health preferred.
• Strong communication and interpersonal abilities, patient-centered care.

Responsibilities

• Patient Assessment: evaluate patients' physical abilities and develop individualized treatment plans.
• Therapeutic Interventions: apply manual therapy, therapeutic exercises, and gait training to improve mobility and pain.
• Functional Training: teach compensatory strategies and assistive device use for independence.
• Patient Education: instruct patients and families on body mechanics and home exercise programs.
• Collaboration: work with physicians, nurses, and other professionals on care plans.
• Documentation: maintain accurate evaluations, treatments, progress notes, and discharge summaries.
• Compliance: follow HIPAA and Medicare regulations.

Responsibilities:

• Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
• Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management.
• Functional Training: Assist patients in developing compensatory strategies, adaptive techniques, and assistive device training to enhance their independence in activities of daily living.
• Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long-term health and well-being.
• Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.

Requirements:

• Licensure: Current and valid Physical Therapist license in the state of Florida.
• Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health or a related setting.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
