Millennium Physician Group is seeking a dedicated Physical Therapist to join our Home Health Team in Sarasota County, FL. You will evaluate patients, develop individualized plans, and provide interventions to improve mobility, strength, balance, and functional independence in patients’ homes.

You will collaborate with physicians, nurses, and other professionals, educate families on rehabilitation and injury prevention, and maintain compliant clinical documentation to deliver high-quality care.

Qualifications

• Florida PT licensure required.
• Minimum 1 year of clinical PT experience, preferably home health.
• Must have strong communication and interpersonal skills; patient-centered approach.
• Reliable transportation to visit patients in their homes.
• Professionalism and commitment to high-quality care.

Responsibilities

• Perform comprehensive patient assessments and develop individualized treatment plans.
• Deliver therapeutic interventions including manual therapy, therapeutic exercises, and gait training.
• Educate patients and families on home exercise programs and safety.
• Collaborate with an interdisciplinary care team to create comprehensive care plans.
• Maintain accurate documentation of evaluations, plans, progress, and discharge summaries.
• Ensure compliance with HIPAA, Medicare, and other regulatory standards.
