Always Best Care Senior Services - Treasure Coast is seeking Physical Therapists for flexible and competitive per-visit pay. This role involves delivering in-home therapy services and conducting evaluations and assessments to ensure patient safety and improvement.

Responsibilities

• Delivering skilled, compassionate in‑home physical therapy services.
• Conducting comprehensive initial evaluations, ongoing reassessments, and detailed fall risk assessments.
• Designing and executing individualized treatment plans focused on balance, strength, and spatial safety.
• Empowering patients and their multi‑generational caregivers through tailored home exercise program education.
• Ensuring accurate, compliant clinical documentation and collaborating smoothly with our interdisciplinary care team.

What We Are Looking For

• Active Hawaii license as a Physical Therapist (PT).
• Minimum of one year of clinical experience (home health experience is a huge plus, but we welcome strong clinical backgrounds).
• Excellent communication, time management, and independent clinical documentation skills.
• Valid driver’s license and reliable transportation to travel to patient homes.
• A genuine dedication to serving seniors and giving back to our local Oʻahu community.
