Job summary

Always Best Care Senior Services - Treasure Coast is seeking a licensed Physical Therapist (PT) to provide in-home care on a per diem basis on O‘ahu, HI. The role offers flexible scheduling, per-visit compensation, and opportunities to build a personalized caseload serving local seniors and kupuna.

Qualifications

• Active Hawaii license as a Physical Therapist (PT).
• Minimum of one year of clinical experience (home health experience is a plus).
• Excellent communication, time management, and independent clinical documentation skills.
• Valid driver’s license and reliable transportation to travel to patient homes.
• Genuine dedication to serving seniors and local O`ahu community.

Responsibilities

• Deliver skilled, compassionate in-home physical therapy services.
• Conduct comprehensive initial evaluations, ongoing reassessments, and detailed fall risk assessments.
• Design and execute individualized treatment plans focused on balance, strength, and spatial safety.
• Empower patients and their multi-generational caregivers through tailored home exercise program education.
• Ensure accurate, compliant clinical documentation and collaborate smoothly with our interdisciplinary care team.

What We Are Looking For

• Active, Hawaii license as a Physical Therapist (PT).
• Minimum of one year of clinical experience (home health experience is a huge plus, but we welcome strong clinical backgrounds).
