Home Health Physical Therapist (PT) / Physical Therapist Assistant (PTA) - Per Diem / PRN

Pay: PT: $80 per visit; PTA: $55-$60 per visit. Location: O‘ahu, HI (reliable transportation required). Schedule: Flexible / Per Diem (PRN) - Build Your Own Caseload.

We are currently hiring licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) who are passionate about providing high-quality, patient-centered home health care.

Responsibilities

• Provide skilled in-home physical therapy services
• Complete evaluations, reassessments, and fall risk assessments (PT)
• Implement individualized treatment plans to improve mobility, balance, strength, and safety
• Educate patients and caregivers on home exercise programs and fall prevention
• Document accurately and communicate patient progress with the interdisciplinary care team
• Maintain compliance with Medicare home health documentation requirements

Requirements

• Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
• Minimum one year of clinical experience (home health experience preferred)
• Strong documentation, communication, and time-management skills
• Ability to work independently in the field
• Valid driver\'s license and reliable transportation
