Job summary

Consonus Healthcare is seeking a Weekend Physical Therapist to join our Valley Hi Nursing Home and Rehabilitation team. This role emphasizes patient-centered care, delivering therapy to seniors, and advancing functional goals through evidence-based plans.

Ideal candidates are licensed in the state or pursuing licensure, possess CPR certification, and demonstrate strong organizational skills. Weekend availability Thursday through Sunday is preferred, with flexible scheduling as needed.

Qualifications

• Graduate of an accredited program with a degree in Physical Therapy.
• Current state licensure - or in process to obtain appropriate licensure.
• CPR Certification (requirement varies by state/location).
• National Provider Identifier (NPI) required upon hire.
• Excellent organizational skills to facilitate program development.

Responsibilities

• Weekend Availability Preferred (Thursday-Sunday): Coverage to complete evaluations for weekend admissions.
• Apply a transformative approach to therapy, crafting personalized care plans.

Responsibilities:

• Weekend Availability Preferred (Thursday-Sunday): Coverage is needed to complete evaluations for weekend admissions. Thursday through Sunday availability is ideal, though scheduling flexibility may be available.
• Apply a transformative approach to therapy, leveraging clinical data to craft personalized care plans that align with your core strengths as a therapist
