LiveWell Homecare Agency, LLC is seeking a Home Health Physical Therapist to deliver personalized in-home physical therapy, enhancing strength, mobility and independence. The role includes planning, documenting, and coordinating care with physicians and families.

Responsibilities include supervising aides, documenting progress, and communicating patient outcomes to care teams and physicians, while ensuring compliance with treatment plans.

Formación

• Current, unrestricted Physical Therapy license.
• Experience collaborating with healthcare professionals.
• At least one year of relevant experience in home health or similar setting.

Responsabilidades

• Planning and administering prescribed treatment plans in patients’ homes.
• Assessing and documenting physical strengths to support clinical evaluation.
• Developing and implementing rehabilitation programs aligned with physician plans and patient goals.
• Providing clear instructions to PTAs and Home Health Aides, overseeing their work and documentation.
• Documenting patient progress, interventions, and evaluations thoroughly and promptly.
• Reporting patient progress to physicians, care teams, patients, and families, and submitting required documentation.
