Job summary

Julian's Heart Homecare LLC is seeking a Home Care Physical Therapist to evaluate patients in their homes, develop personalized treatment plans, and deliver one-on-one therapy to aid recovery after surgery, injury, or chronic illness.

The role involves starting with initial assessments, creating home-based care plans under physician direction, and guiding patients through therapeutic interventions and safety improvements in their living spaces.

Qualifications

• DPT from an accredited program.
• Minimum of 1 year of clinical or home health care experience.
• Valid driver’s license, reliable automobile, active auto insurance, and willingness to drive to patients’ homes.
• Effective written and verbal communication.
• Ability to stand, bend, lift and transfer patients (up to 50–100 lbs).
• US Citizen or Green Card holder.
• Current, active, and unrestricted Physical Therapist state license.

• Therapeutic Interventions:Provide gait and balance training, muscle strengthening, pain management, and instruction on using assistive devices (walkers,canes).
• Home SafetyAssessments:Inspect the patient's living environment for safety hazards and recommend necessary modifications or accessibility tools.

Qualifications
