- Provides physical therapy services to patients according to a written physician's plan of care, including assessment and evaluation of therapeutic, rehabilitative, and functional status, and participation in developing the total plan of care. Evaluates home environment.
- Assesses muscle strength, mobility, gait, ROM, and potential for rehab.
- Directs physical therapy treatment.
- Instructs patients and families/caregivers in the use and care of therapeutic appliances.
- Determines priority needs for physical therapy.
- Reports to the physician on patient’s reaction to treatment or changes in condition.
- Initiates physical therapy programs and instructs other personnel and/or family/caregivers in phases of therapy, participating in case conferences.
- May train patients in the use of prosthetic devices.
- Identifies patient and family/caregiver needs for other home health services and makes referrals as necessary.
- Prepares and submits clinical and progress summaries based on goal attainment.
- Participates in discharge planning for patients.
- Provides physical therapy consultation to home families/caregivers when indicated.
- Provides in-service education programs for nursing personnel as needed.
- Participates in peer consultation processes.
- Supervises physical therapy assistants according to policy and regulations.
- When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care biweekly.
Qualifications
- Possesses a degree from an accredited physical therapy program.
- Licensed to practice as a physical therapist within the state.
- At least two years of relevant experience, including community/home health.
- Good verbal and written communication skills, organized.
- Current CPR certification.
