Job summary

Ultimate Therapy, a rehabilitation provider serving Autumn Lake at Glen Hill in Danbury, CT, is hiring a full-time Physical Therapist (PT). You will perform comprehensive PT evaluations, create individualized treatment plans, and guide residents to improve mobility and independence in a skilled nursing/post-acute setting.

The role emphasizes collaboration with a multidisciplinary team, documentation, and patient/caregiver education. Connecticut PT license required; new grads encouraged to apply.

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong communication and clinical decision‑making skills.
• Post-acute care or skilled nursing experience preferred.
• New graduates are encouraged to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop individualized treatment plans based on resident needs and goals.
• Provide therapy focused on strength, balance, gait, and mobility.
• Monitor resident progress and modify treatment plans as needed.
• Educate residents, families, and caregivers on exercises and safe mobility.
• Collaborate with PTAs, OTs, COTAs, SLPs, nursing and interdisciplinary team.
• Complete accurate and timely clinical documentation.
