Ultimate Therapy Services in Danbury, CT is seeking a Physical Therapist (PT) for a full-time role at Autumn Lake at Glen Hill. You will evaluate residents, develop individualized treatment plans, and provide skilled therapy to improve strength, balance, gait, and mobility in a collaborative skilled nursing and post-acute setting.

The position emphasizes teamwork with PTAs, OTs, COTAs, SLPs, and nursing, along with ongoing documentation and resident education.

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong communication and clinical decision-making skills.
• Skilled nursing or post-acute experience preferred but not required.
• New graduates are encouraged to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations
• Develop individualized treatment plans based on resident needs and functional goals
• Provide skilled therapy focused on strength, balance, gait, mobility, transfers, and endurance
• Monitor resident progress and modify treatment plans as appropriate
• Educate residents, families, and caregivers on exercises and safe mobility techniques
• Collaborate with PTAs, OTs, COTAs, SLPs, nursing, and other members of the interdisciplinary team
• Complete accurate and timely clinical documentation
