Job summary

Visalia Post Acute is seeking a skilled Physical Therapist (PT) to join our rehabilitation team at Visalia, CA. This full-time position focuses on restoring function and independence for residents in a skilled nursing and short-term rehab setting.

The PT evaluates and treats residents, develops individualized plans, collaborates with physicians and the interdisciplinary team, and maintains accurate documentation. Leadership and education duties are also included.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Master's or Doctorate degree in Physical Therapy preferred.
• California license in good standing required.

Responsibilities

• Evaluate residents within 24 hours (or one business day) of physician referral and document delays.
• Develop and implement individualized treatment plans; obtain physician approval for services.
• Provide PT treatment per plan of care and supervise PTAs, aides, and students as needed.
• Communicate resident progress and care plans with the rehab and interdisciplinary teams.
• Complete evaluations, treatment notes, progress notes, and discharge documentation per requirements.
