Job summary

Visalia Post Acute is seeking a motivated Physical Therapist Assistant (PTA) to join our therapy team in Visalia, CA. The role supports mobility and quality of life for residents under a physician-directed plan.

Holders of an active CA PTA license will work closely with the Physical Therapist and interdisciplinary staff to ensure compliant care, accurate documentation, and effective discharge planning in a regulated healthcare setting.

Qualifications

• Associate Degree from an accredited PTA program is required.
• Experience in a skilled nursing facility is preferred but not required.
• Proficiency with EMR systems such as Net Health and PCC is preferred.

Responsibilities

• Treat residents as directed by the Physical Therapist.
• Complete timely, accurate documentation in accordance with regulations.
• Maintain a safe treatment environment following infection control protocols.
• Participate in resident care conferences and rehabilitation meetings.
• Communicate with the supervising PT and interdisciplinary team about progress and discharge planning.
• Educate residents and families on treatment plans and home exercise programs.
• Assist in obtaining durable medical equipment as needed.
• Support staff training and protect resident confidentiality.
