Evolve Therapy Services in Olivia, MN is hiring a Physical Therapist for an in-house Skilled Nursing Facility. You will provide PT services, assess residents, and develop treatment plans to improve function and quality of life.

The role requires a BS in Physical Therapy, an active PT license, and ongoing continuing education. Full-time, part-time, and PRN options are available, with a $5,000 sign-on bonus and tuition reimbursement for eligible candidates.

Qualifications

• Minimum BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to CMS expectations and compliant documentation.

Responsibilities

• Provide PT services to residents per standards and procedures.
• Document and bill PT services per company procedures.
• Evaluate residents and develop treatment plans with goals.
• Conduct screenings to determine need for intervention.
• Communicate evaluation findings and plans to residents and team.
• Complete compliant documentation in NetHealth EMR.
