Qualifications

• New Jersey license or eligibility to practice.
• Experience in long-term care, specifically skilled nursing facilities.
• Strong organizational, interpersonal and communication skills.
• Ability to manage patients with different personalities and ability levels.
• Creative problem solving skills.

Responsibilities

• Collaborate with therapy teams and Rehab Department.
• Complete evaluations and all necessary documentation.
• Provide recommendations supporting communications/assessments.
• Provide therapy as outlined in treatment plans and prescriptions.
• Independently manage workflow including treatment documentation of individual and group treatments and notes in the electronic records system.
• Align therapy treatments and plans with understanding of PDPM and Medicaid/Medicare billing.
