MB Healthcare is seeking a Physical Therapist for a local Skilled Nursing Facility. The role involves collaborating with the Rehab Department and therapy teams at Stratford Manor Rehabilitation & Care Center to deliver patient-centered therapy.

The ideal candidate holds a New Jersey license and has experience in long-term care, with strong organizational and communication skills to manage diverse patient needs and documentation requirements.

Qualifications

• New Jersey licensed Physical Therapist.
• Experience in long-term care (Skilled Nursing Facility) preferred.
• Strong organizational, interpersonal and communication abilities.
• Ability to manage patients with varying personalities and levels of function.

Responsibilities

• Collaborate with therapy teams and Rehab Department.
• Complete evaluations and documentation.
• Provide recommendations supporting communications/assessments.
• Deliver therapy per treatment plans and prescriptions.
• Assist patients to improve functional abilities.
• Manage workflow including treatment documentation in EHR.
• Familiar with PDPM and Medicaid/Medicare billing.

Description

Local Skilled Nursing Facility Now seeking a Physical Therapist

Responsibilities/duties:

• Work collaboratively with the therapy teams and Rehab Department
• Complete evaluations and all necessary documentation
• Provide recommendations supporting communications/assessments
• Provide therapy as outlined in treatment plans and prescriptions
• Offer treatments that help improve patient’s inabilities and/or challenges
• Independently manage workflow which includes allotting time for treatment documentation of individual and group treatments and notes in the electronic records system
• Familiar with PDPM
• Align therapy treatments and plans with understanding of Medicaid and Medicare billing

Qualifications:

• New Jersey License
• Background/experience in long term care (specifically a Skilled Nursing Facility)
• Strong organizational, interpersonal and communication skills
• Ability to manage patients with different types of personalities and ability levels
• Creative problem solving skills
