Qualifications

• Bachelor's degree or higher in Physical Therapy.
• Active state license.
• 2+ years of experience in a skilled nursing facility.

Responsibilities

• Screen/evaluate patients with cognitive/ADL impairments and develop plan of care.
• Provide rehabilitative, skilled therapies per clinical requirements.
• Administer therapies including ADL retraining, exercises and modalities.
• Document treatment minutes and progress in medical records.
• Communicate progress with supervisor and care team; discharge planning as needed.
