Job summary

Evolve Therapy Services is seeking a Physical Therapist to work with residents at an in-house Skilled Nursing Facility. The role focuses on planning, organizing, and directing Physical Therapy services in accordance with applicable standards and CMS expectations. Must hold a PT license and a BS in Physical Therapy.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screening of residents at regular intervals to determine need for intervention/treatment.
• Develop treatment plans with short- and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings, treatment plan and recommendations to residents and families.
