Qualifications

• Bachelor's Degree in Physical Therapy is required.
• Licensed Physical Therapist (IDFPR) in Illinois is required.
• BLS certification within 30 days (AHA) required.
• Attend and satisfactorily complete required continuing education on acute stroke care.

Responsibilities

• Perform safe patient transfers with assistance as needed, using proper body mechanics.
• Educate patients, families, caregivers, students, and other health care providers.
• Design and adjust the plan of care with goals, treatment, outcomes, and discharge planning.
• Evaluate clinical findings to determine PT diagnoses and outcomes.
• Document examinations and care per CTSCP67 policy.
• Screen patients to determine the need for therapy services and use support personnel appropriately.
• Communicate patient progress and discharge needs with the medical team.
