Job summary

Trinity Healthcare Talent Services is seeking a Physical Therapist for a 86-bed Skilled Nursing Facility. The role focuses on helping residents recover mobility and reduce pain through individualized treatment plans and integrated therapies in a supportive, multidisciplinary team.

Responsibilities include initial evaluations, progress tracking, documentation, patient and family education for home exercise programs, and staying current with PT advances.

Qualifications

• PT degree from an accredited program and valid state license.
• Minimum of 2 years of experience in a similar SNF setting preferred.
• Strong knowledge of anatomy, physiology, and kinesiology.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Passion for helping others and improving residents' quality of life.

Responsibilities

• Conduct initial evaluations and develop personalized treatment plans.
• Utilize therapeutic techniques and modalities to improve function and reduce pain.
• Monitor and document patient progress, adjusting plans as needed.
• Collaborate with physicians, nurses, and occupational therapists for comprehensive care.
• Educate patients and families on exercises to continue progress at home.
• Maintain accurate patient records and comply with standards.
• Stay current with advances in physical therapy and pursue continuing education.
