Job summary

Trinity Healthcare Talent Services is seeking a dedicated Physical Therapist for a 193-bed skilled nursing facility. The PT will assess and treat patients to restore strength, mobility, and independence, in a setting with a mix of short-term and long-term care.

The role requires evaluating patients, developing individualized plans, implementing therapies, documenting progress, and collaborating with the care team.

Qualifications

• Licensed or eligible to obtain state licensure in physical therapy.
• 1–2 years of experience in skilled nursing or long-term care preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct thorough evaluations of patients to assess physical abilities and limitations.
• Develop individualized treatment plans based on patient needs and goals.
• Implement and monitor treatment plans, adjusting as needed for progress.
• Utilize various therapeutic techniques to improve function and reduce pain.
• Educate patients and families on exercise techniques for home use.
• Document all treatments and progress in medical records.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Stay updated on new PT techniques and advancements.
