Job summary

Nexus Health is seeking a Physical Therapist to develop individualized treatment plans for residents to regain independence and safely participate in daily activities, while enhancing quality of life.

You will assess evaluations, create rehabilitation programs, administer prescribed therapies, and collaborate with a supportive team in a SNF/long-term care setting. Illinois license required, with preference for experience in long-term care.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
