Job summary

Nexus Health in Belleville, IL is seeking a licensed Physical Therapist to develop individualized treatment plans to help residents regain independence and safely participate in daily activities, all while enhancing quality of life.

The role involves assessing evaluations, creating personalized rehab programs, and administering prescribed PT treatments to relieve pain and restore function in a supportive, team-oriented environment.

Qualifications

• Active Illinois PT license required.
• Experience in SNF/long-term care is preferred.
• Knowledge of current physical therapy treatment practices.
• Familiarity with PCC Electronic Documentation System is a plus.

Responsibilities

• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

Skills

Active Illinois PT license

SNF/Long term care experience
