Qualifications

• Active Illinois license in Physical Therapy.
• SNF/Long-term care experience preferred.
• Current knowledge of standard treatment practices.
• PCC Electronic Documentation System familiarity preferred.

Responsibilities

• Assess patient evaluations and test results and interpret them.
• Develop individualized treatment plans using varied techniques.
• Create personalized rehabilitation programs for residents.
• Administer medically prescribed physical therapy treatments.
