Job summary

Nexushm in Illinois is seeking a dedicated Physical Therapist to support residents in regaining independence and safely participating in daily activities. You will develop individualized treatment plans and create personalized rehabilitation programs to enhance quality of life.

Responsibilities include evaluating patient data, selecting diverse treatment techniques, and administering prescribed PT therapies to relieve pain and restore function.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Assess patient evaluations and test results.
• Develop treatment plans using a variety of treatment techniques.
• Create personalized rehabilitation programs for patients.
• Administering PT treatments to relieve pain and improve or restore function.
