Job summary

Pate Rehabilitation is seeking a dedicated Physical Therapist for Fort Worth, TX to perform evaluations and provide patient-centered therapy. You will develop and progress treatment plans based on physician orders and patient goals, while collaborating with the care team to optimize outcomes.

The role requires a state license, BLS/CPR certification, and strong communication/organization skills. Prior neuro rehab experience is preferred, with opportunities for ongoing professional development.

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the state of operation.
• Current Basic Life Support (BLS) or CPR certification per state regulations.
• A minimum of one year of work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to neurological injuries.
• Communicates effectively and professionally in verbal and written interactions.
• Demonstrates strong organization skills and attention to detail.
• Possesses the physical ability to perform duties (sitting, lifting, standing, bending, stooping, stretching, walking, pushing, pulling).

Responsibilities

• Performs PT evaluation and treatment based on physician orders/referrals.
• Completes comprehensive evaluation using appropriate tools and procedures.
• Reviews patient records for diagnosis, medical history, precautions and contraindications.
• Sets measurable goals and develops treatment plans for independence.
• Re-assesses progress and adjusts goals or treatment as needed.
• Communicates patient needs and progress to care team and family.
• Consults with medical providers on evaluation, progress, and discharge.
• Recommends adaptive equipment to improve function within rehab program.
• Maintains and submits evaluation, treatment, and billing documentation as required.
• Adheres to company policies, HIPAA, and regulatory standards.
• Supervises PTAs and PT aides in direct patient care.
• Attends staff development programs and ongoing education per policy.
