Job summary

Work Overview

We are looking for a Physical Therapist who will be responsible for the delivery of quality patient care according to the patients individual physician orders. Patient care may involve evaluations, administering prescribed treatments and therapies, care planning, communication with other members of the patients multi-disciplinary team, coordination of care, health promotion and teaching and training family members.

Key Responsibilities

• Provide Physical Therapy services as prescribed by the patients physician
• Maintain a full-time caseload of at least 33 visits
• Provide a written evaluation defining Physical Therapy needs, goals and expected outcomes in conjunction with referring physicians plan of treatment
• Follow policies and procedures and adheres to all applicable patient care standards, licensure regulations and state laws
• Develop a written plan of care for Physical Therapy with reasonable goals and outcomes
• Maintain and adjust the plan of care as needed for assigned patients according to identified Physical Therapy goals and outcomes
• Maintain patient and company confidentiality
• Maintain effective communication between staff, healthcare team members, and family
• Participate in coordination of services (including discharge planning), with health care team, physicians, patient/guardians, nurses and others

Benefits

Benefits are offered to full time employees who work 30 hours or more a week. Full time employees are eligible for benefits at 90 days of employment.
