• Completion of continuing education related to acute stroke care.
• Demonstrated ability to collaborate with patients and care teams.

Responsibilities

• Assess and plan patient care, adjusting goals and interventions based on response and evidence.
• Educate patients and families using effective teaching methods.
• Perform examinations and determine PT diagnoses and outcomes of care.
• Document findings per CTSCP67 and regulatory standards.
• Supervise PT aides and rehab technicians in accordance with regulations.
• Communicate discharge plans and equipment needs with medical team.
