Qualifications

• Ability to work independently in a home-based setting.

Responsibilities

• Provide skilled Physical Therapy services in the patient's home.
• Evaluate patients and develop individualized plans of care.
• Carry out established treatment plans and interventions.
• Provide functional activities, therapeutic exercises, ADL training, strengthening and coordination activities.
• Monitor patient progress and communicate changes to supervising therapist and clinical team.
• Maintain accurate and timely clinical documentation per regulatory requirements.
• Communicate professionally with patients, caregivers, therapists, and office staff.
• Provide one-on-one therapy services in the patient's home.

Homeside Rehab is seeking compassionate and motivated Physical Therapist (PT) or Physical Therapy Assistant (PTA) to provide one-on-one home-based therapy services in Sundown, NY 12740.

Job Responsibilities

• Provide skilled Physical Therapy services in the patient's home

• Evaluate patients and develop individualized plans of care, as applicable to your professional scope of practice

• Carry out established treatment plans and therapeutic interventions

• Provide functional activities, therapeutic exercises, ADL training, strengthening, coordination activities, and other interventions within your professional scope

• Monitor patient progress and communicate changes or concerns to the appropriate supervising therapist and clinical team

• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements

• Communicate professionally with patients, caregivers, therapists, and office staff

• Provide one-on-one therapy services in the patient's home

Job Qualifications

• Active New York State Physical Therapist (PT) or Physical Therapy Assistant (PTA) license/certification, as applicable

• Valid driver's license and reliable transportation for home visits
