Job summary

Interim HealthCare Inc. is seeking a Home Health Physical Therapist to deliver patient-focused care in Bradford County, PA. This role offers a flexible schedule, 1:1 therapist-to-patient ratios, and the autonomy to manage assignments while balancing your personal life.

You will work with a home health team to achieve patient goals, observe movements, and adjust care plans. PA license, 2+ years of PT experience, CPR certification, and regulatory knowledge are required.

Qualifications

• Must hold a Doctor of Physical Therapy (DPT) degree and PA PT license.
• Minimum 2 years of physical therapy experience, ideally in home health.
• CPR certification and knowledge of state/federal home health regulations.

Responsibilities

• Provide home-based physical therapy to patients who have mobility and strength limitations.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) on the plan of care and goals.
• Observe movements, document progress, and adjust the care plan as needed.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed

  A few must-haves for Home Health Physical Therapists:

  • Graduate of an accredited Physical Therapist Program and active PT license in PA
  • Minimum of 2years of physical therapy experience, ideally in home healthcare
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, dexterity and compassion
