Job summary

Interim HealthCare of Mercer County is seeking part-time Home Health Physical Therapists (PTs) to deliver one-to-one, patient-centered care in home settings. You will assess, plan, and implement personalized therapy to restore strength and mobility, collaborating with a broader home health team to meet patient goals.

Qualifications

• Graduate of an accredited Physical Therapist Program with active PT license in New Jersey.
• Minimum of 2 years of physical therapy experience, ideally in home healthcare.
• CPR certification is required.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based physical therapy to patients who have mobility, strength and functional limitations.
• Collaborate with a home health team (RN, LPN, CNA, HHA, OT, SLP) based on the patient’s plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain.
• Educate patients on their plan of care, goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safe assistance with exercises.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapists:

• Graduate of an accredited Physical Therapist Program and active PT license in New Jersey
