Job summary

Interim HealthCare of North Central Pennsylvania is seeking a Home Health Physical Therapist (PT) to provide patient-centered care in Bradford County. You’ll assess mobility, design plans of care, and work with a multi-disciplinary home health team to restore function and independence.

You’ll enjoy a flexible schedule, per-visit pay, and opportunities for growth through online training and CEUs. A PA PT license and CPR certification are required, with relevant home health experience preferred.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PA PT license.
• Minimum of 2 years of physical therapy experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Good clinical judgment, strong interpersonal skills, dexterity and compassion.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality.
• Work as part of a home health team focused on the patient’s plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plan as needed.

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed

  A few must-haves for Home Health Physical Therapists:

  • Graduate of an accredited Physical Therapist Program and active PT license in PA
