Concept Rehab is seeking a Physical Therapist to join our skilled nursing and post-acute care team in Centerville, OH. You will evaluate patients, design individualized treatment plans, and deliver evidence-based therapies to restore mobility and independence.

Requirements

What You'll Do

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans
• Deliver evidence-based interventions focused on mobility, balance, strength, pain management, and functional independence
• Monitor patient progress and adjust treatment strategies to maximize outcomes
• Collaborate with physicians, nursing staff, families, and interdisciplinary care teams
• Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations
• Complete timely, accurate, and compliant clinical documentation
• Educate patients and caregivers on safe mobility techniques and discharge recommendations
• Participate in care conferences, team meetings, and quality improvement initiatives
• Promote a positive patient experience through compassionate and patient-centered care

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Ohio Physical Therapist license in good standing
• Skilled nursing, post-acute, or geriatric rehabilitation experience preferredli>
• Excellent communication and clinical reasoning skills
• Ability to build strong relationships with patients, families, and healthcare professionals
