Job summary

Human Touch Home Health in Silver Spring, MD is hiring Physical Therapists (PTs) for Full-Time, Part-Time, and PRN home health opportunities across the Silver Spring area. You will assess patients in their homes, develop plans of care, and help them improve mobility and balance while navigating stairs. Active Maryland PT license is required.

Graduate of an accredited PT program. Reliable transportation and strong communication skills are essential; WellSky experience is a plus and mileage

Qualifications

• Active Maryland Physical Therapist License Required.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation.
• Strong clinical and communication skills.
• Ability to independently manage home health visits.
• Home Health Experience Preferred, but Not Required.
• WellSky Experience is a Plus.

Responsibilities

• Complete comprehensive PT evaluations in patients' homes.
• Develop individualized plans of care.
• Help patients improve strength, balance, endurance, and mobility.
• Provide gait and transfer training.
• Develop therapeutic exercise programs.
• Address fall risks and home safety concerns.
• Help patients safely navigate stairs and their home environment.
• Educate patients, families, and caregivers.
• Monitor progress and adjust treatment plans.
• Complete reassessments and discharge planning.
• Collaborate with physicians, nurses, OTs, SLPs, and other care team members.
• Complete accurate and timely documentation using WellSky.
