Job summary

Human Touch Home Health is seeking Physical Therapists to cover home health in Rockville and Gaithersburg, Maryland. We offer full-time, part-time, and PRN opportunities with flexible scheduling and focused territories.

Join our team for one-on-one patient care, mileage reimbursement, and benefits for eligible full-time employees. Home health experience is preferred but not required; WellSky experience is a plus.

Qualifications

• Active Maryland PT license required.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation to patient homes.

Responsibilities

• Perform comprehensive physical therapy evaluations
• Develop individualized plans of care
• Provide therapeutic exercises and interventions
• Improve strength, balance, endurance, and functional mobility
• Provide gait, transfer, and stair training
• Assess fall risks and home safety concerns
• Help patients regain mobility after illness, surgery, injury, or hospitalization
• Educate patients and caregivers
• Monitor progress and modify treatment plans
• Complete reassessments and discharge planning
• Collaborate with the interdisciplinary team
• Complete accurate documentation using WellSky

We have patient needs throughout both areas and are looking for therapists who want the flexibility of home health without being expected to drive all over Montgomery County.

What You'll Do

• Active Maryland Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Reliable transportation
• Strong assessment and treatment skills
• Excellent communication and documentation abilities
• Ability to work independently in patients' homes
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
