Job summary

Holy Cross Home Care and Hospice (Maryland) seeks a dedicated Physical Therapist to deliver home-based rehabilitation services. You will assess patients, design and adjust treatment plans, and provide hands-on therapy in patients' homes.

The role emphasizes independent work, strong documentation, and collaboration with an interdisciplinary team within Trinity Health At Home’s network.

Qualifications

• Current Maryland Physical Therapist license
• Graduate of an accredited physical therapy program
• At least 1 year of clinical/home care experience
• Valid driver’s license and reliable transportation
• Ability to work independently and manage time effectively

Responsibilities

• Provide professional physical therapy services as prescribed by physicians, following state practice acts and organizational policies.
• Assess patient conditions, develop individualized treatment plans, and revise care plans as needed.
• Implement therapeutic and rehabilitative procedures, including manual therapy, exercises, and physical agent modalities.
• Educate patients and caregivers on therapy programs and home exercises.
• Collaborate with interdisciplinary teams to ensure coordinated, effective care.
• Maintain accurate documentation and participate in quality improvement initiatives.

Position Purpose:

What You Will Do:

Minimum Qualifications:

• Current licensure as a Physical Therapist in the State of Maryland
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience (home care experience preferred).
• Valid driver’s license and reliable transportation.
