Job summary

Holy Cross Home Care Team, part of Trinity Health At Home, is seeking a licensed Physical Therapist to deliver in-home therapy across Prince George’s County, MD. You will assess patients, create and adjust personalized plans, and collaborate with an interdisciplinary team to ensure high-quality, compassionate care.

• Educate patients and caregivers on therapy programs and home exercises.
• Collaborate with interdisciplinary teams to ensure coordinated, effective care.
• Maintain accurate documentation and participate in quality improvement initiatives.

Position Purpose:

What You Will Do:

Minimum Qualifications:

• Current licensure as a Physical Therapist in the State of Maryland
• Graduate of an accredited physical therapy program.
• At least one year of clinical experience (home care experience preferred).
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.
