Concept Rehab is seeking a Physical Therapist (PT) – PRN to join our therapy team in Waterford, MI. This flexible PRN opportunity supports skilled nursing and rehabilitation, with scheduling that can fit around other professional and personal commitments.

As part of a therapist-owned team with over 45 years’ experience, you’ll evaluate patients, develop individualized plans, and provide interventions to improve strength, mobility, balance, and functional independence.

Responsabilidades

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination and functional independence
• Monitor patient progress and modify treatment plans as needed
• Collaborate with PTAs and other members of the interdisciplinary team
• Educate patients, families and caregivers
• Participate in care planning and discharge planning
• Complete accurate and timely clinical documentation
