Concept Rehab is seeking a PRN Physical Therapist (PT) to join our team in Bay City, Michigan. This flexible PRN opportunity offers meaningful, patient-centered care in a supportive rehabilitation environment.

The PT will evaluate patients, develop individualized treatment plans, and collaborate with the healthcare team to improve mobility, strength, balance and function. We value compassionate, patient-focused clinicians and provide CEU support and competitive compensation.

Qualifications

• Current Physical Therapist license in Michigan.
• Graduate of an accredited Physical Therapy program.
• Current or eligible Michigan PT licensure.
• Strong clinical, communication, and organizational skills.
• Compassionate, patient-focused approach.
• Experience in skilled nursing, rehabilitation or post-acute settings is a plus, but not required.

Responsabilités

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve mobility, strength, balance and function.
• Monitor patient progress and adjust treatment plans as needed.
• Document evaluations, treatments and progress accurately and timely.
• Collaborate with PTAs, nursing staff, physicians and other members of the healthcare team.
• Help patients achieve greater independence and quality of life.
