Job summary

Concept Rehab is seeking a Physical Therapist (PT) – PRN to join our therapy team in Waterford, MI. This flexible PRN opportunity supports skilled nursing and rehabilitation services with a therapist-owned and operated platform.

You will evaluate patients, develop individualized plans, and provide skilled PT interventions to improve strength, mobility, balance and functional independence. Collaboration with the interdisciplinary team and education for patients and families are key parts of the

Qualifications

• PT license or eligibility to practice in Michigan.
• Experience in skilled nursing and rehabilitation settings preferred.
• Ability to develop individualized treatment plans and adjust as needed.

Responsibilities

• Evaluate patients and develop individualized treatment plans.

• Monitor progress and modify treatment plans as needed.
