Job summary

Concept Rehab is seeking PRN Physical Therapists to support multiple skilled nursing and rehabilitation facilities in the Dayton, Ohio market. You’ll evaluate residents, develop individualized plans, and deliver skilled PT services in a flexible schedule.

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Physical Therapy.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, function, and safety.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Supervise PTAs as required and participate in care planning/discharge planning.
• Document accurately and complete billing; educate patients and staff on maintenance and restorative programs.

We are currently seeking PRN Physical Therapists (PTs) to support multiple skilled nursing and rehabilitation facilities throughout the Dayton, Ohio market.

Opportunities Are Available In

• Dayton
• Kettering
• Centerville
• Yellow Springs
• Waynesville

Candidates are not expected to work at all locations. Facility assignment is based on applicant preference, scheduling availability, and geographic convenience.

This position supports a network of skilled nursing and rehabilitation facilities throughout the greater Dayton area. During the interview process, our recruiting team will discuss preferred work locations and travel expectations with each candidate.

Requirements

What You’ll Do

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, function, and quality of life
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members
• Supervise Physical Therapist Assistants (PTAs) as required
• Participate in care planning and discharge planning
• Recommend appropriate equipment and interventions to support patient outcomes
• Complete timely, accurate documentation and billing
• Educate patients, families, and facility staff on maintenance and restorative programs

Qualifications

• Bachelor's, Master's, or Doctorate Degree in Physical Therapy
• Current Physical Therapist license in the State of Ohio
• Strong communication and clinical decision-making skills
• Commitment to providing compassionate, patient-centered care
