Concept Rehab is seeking a PRN Physical Therapist (PT) to join our Bay City, Michigan team. This flexible PRN opportunity supports meaningful patient care in a collaborative rehabilitation environment.

Qualifications

• Current Physical Therapist license in Michigan or eligible for licensure.
• Graduate of an accredited Physical Therapy program.
• Strong clinical and communication skills with organized approach.
• Compassionate, patient-focused mindset; post-acute experience is a plus but not required.

Responsabilités

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled PT interventions to improve mobility, strength, balance and function.
• Monitor progress and adjust treatment plans as needed.
• Document evaluations, treatments and progress timely and accurately.
• Collaborate with PTAs, nursing staff, physicians and other team members.
