Job summary

Elara Caring is seeking a motivated Physical Therapist to join our home-based care team in Adrian/Monroe. You will evaluate patients, design individualized treatment plans, deliver PT services in patients' homes, monitor progress, and update care plans as needed.

Responsibilities include collaboration with physicians and the care team, timely clinical documentation, and supervising therapy assistants as appropriate.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Ability to travel between patient homes and work locations with reliable transportation.
• Ability to lift, push, pull, and move up to 50 pounds.

Job Description

As a Physical Therapist, you’ll contribute to our success in the following ways:

• Evaluate patients and develop individualized treatment plans.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license, maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
