Job summary

Canyon Springs Post Acute in San Jose, CA is seeking a Physical Therapist (PT) to join our in-house rehab team on a PRN basis. Starting pay is $64-65+ per hour, with premium compensation for clinical expertise and flexible scheduling.

You will perform evaluations, develop treatment plans, supervise PTAs and restorative staff, document progress, and collaborate with nursing to support residents' mobility and independence. A CA PT license is required.

Qualifications

• Current and unrestricted California PT license required.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Provide skilled PT treatments in accordance with the California State Practice Act.
• Supervise Physical Therapist Assistants (PTAs) and restorative staff to ensure continuity of care.
• Maintain timely and accurate documentation, including daily treatment notes, progress reports, and recertifications.
• Collaborate closely with nursing staff and the interdisciplinary care team; participate in care planning meetings and utilization reviews as needed.
• Educate residents and families on mobility safety, home exercise programs, and discharge planning.
• Recommend and coordinate durable medical equipment (DME) such as walkers, wheelchairs, and braces.

Key Responsibilities

As a Physical Therapist at Canyon Springs Post Acute, you will play a critical role in helping residents regain strength, mobility, and independence.

Clinical Duties Include:

• Conduct comprehensive evaluations and develop individualized treatment plans based on resident goals and clinical needs
• Provide skilled, evidence‑based physical therapy treatments in accordance with the California State Practice Act
• Supervise Physical Therapist Assistants (PTAs) and support Restorative Nursing Aides to ensure continuity of care
• Maintain timely and accurate documentation, including daily treatment notes, progress reports, and recertifications
• Collaborate closely with nursing staff and the interdisciplinary care team; participate in care planning meetings and utilization reviews as needed
• Educate residents and families on mobility safety, home exercise programs, and discharge planning
