Qualifications

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, and independence.

• Participate in care planning and discharge preparation.
• Educate residents, families, and caregivers.
• Maintain accurate and timely clinical documentation.
