Job summary

Concord Post Acute in Concord, CA is seeking a skilled Physical Therapist (PT) for PRN/per-diem work. You will conduct evaluations, develop individualized plans and collaborate with PTAs to support mobility and independence in a fast-paced, post-acute rehab setting.

The role emphasizes flexibility, clinical autonomy and team-based care within regulatory guidelines. California licensure is required for this PRN position.

Qualifications

• Current California Physical Therapist license in good standing.

Responsibilities

• Perform comprehensive physical therapy evaluations following physician referral.
• Develop and implement individualized treatment plans focused on mobility, balance, strength, pain management and functional independence.
• Supervise and collaborate with PTAs to ensure quality patient care and compliance with state practice standards.
• Participate in interdisciplinary care planning, patient conferences and utilization review meetings.
• Recommend appropriate DME to support safe mobility and discharge planning.
• Educate residents, families and caregivers on exercise programs, safety strategies and long-term maintenance plans.
• Maintain accurate and timely documentation in accordance with regulatory and payer guidelines.

Position Details

• Position: Physical Therapist (PT) - PRN
• Location: Concord, CA
• Compensation: Starting at $64-$65/hour
• Schedule: PRN / Per-Diem
• Environment: Supportive, team-oriented, and fast-paced rehab setting

Key Responsibilities

• Perform comprehensive physical therapy evaluations in a timely manner following physician referral
• Develop and implement individualized treatment plans focused on mobility, balance, strength, pain management, and functional independence
• Supervise and collaborate with Physical Therapist Assistants (PTAs) to ensure quality patient care and compliance with state practice standards
• Participate in interdisciplinary care planning, patient conferences, and utilization review meetings
• Recommend appropriate Durable Medical Equipment (DME) to support safe mobility and discharge planning
• Educate residents, families, and caregivers on exercise programs, safety strategies, and long-term maintenance plans
• Maintain accurate and timely documentation in accordance with regulatory and payer guidelines
