Job summary

Ridge Valley Center for Nursing & Rehabilitation is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a flexible PRN basis. The PTA will work under a licensed Physical Therapist to support residents in improving strength, mobility, and independence in a skilled nursing setting.

Responsibilities include documenting sessions, progress notes, and participating in care conferences while collaborating with the interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current and valid PTA license in the state of practice.
• Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
• Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds when necessary.

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Assist residents with improving mobility, balance, strength, endurance, and functional independence.
• Carry out individualized treatment plans designed to meet resident goals and improve outcomes.
• Accurately document treatment sessions, resident progress, and daily notes in accordance with facility policies and regulatory requirements.
• Monitor resident response to therapy and communicate progress, concerns, and recommendations to the supervising Physical Therapist.
• Assist with discharge planning and educate residents and families on home exercise programs, safety techniques, and continued care.
• Collaborate with nursing staff, physicians, therapists, and interdisciplinary team members to provide coordinated resident care.
• Participate in care conferences, departmental meetings, and quality improvement initiatives.
• Support therapy department needs and provide coverage at nearby affiliated facilities as needed.
