AdvantageCare Rehabilitation is seeking a PRN Physical Therapist (PT) to join the Grand Rapids Care Center rehab team in Grand Rapids, OH. The role offers flexible PRN hours, with opportunities across weekday and weekend shifts, enabling you to balance other commitments while contributing to geriatric rehabilitation.

You will evaluate and treat adults in a skilled nursing setting, collaborate with a multidisciplinary team, and drive patient-centered plans of care in a supportive environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible Ohio Physical Therapist license.
• Strong clinical judgment and communication skills.
• Ability to work collaboratively with PTAs and the interdisciplinary team.
• Geriatric or skilled nursing rehabilitation experience is a plus.
• Patient-first approach to care.

Responsibilities

• Evaluate and treat patients with mobility and functional needs.

• Address strength, balance, gait, transfers, endurance and functional mobility.

What Will You Do as a Physical Therapist?

As a Physical Therapist at Grand Rapids Care Center, you'll evaluate and treat patients with a variety of mobility and functional needs.

Your role may include:

• Performing comprehensive Physical Therapy evaluations
• Developing individualized plans of care based on patient needs and goals
• Addressing strength, balance, gait, transfers, endurance and functional mobility
• Providing therapeutic exercise, neuromuscular re-education and functional mobility training
• Assessing the need for assistive devices and providing appropriate training
• Developing fall-prevention and safety strategies
• Monitoring patient progress and modifying treatment plans when appropriate
• Educating patients, families and caregivers
• Collaborating with PTAs and the interdisciplinary care team
• Completing accurate and timely clinical documentation
• Helping patients work toward greater safety, mobility and independence
