Job summary

AdvantageCare Rehabilitation is seeking a PRN Physical Therapist (PT) to support residents at Valley View in Montoursville, PA. The role offers flexible hours, allowing you to pick up shifts based on availability while maintaining high-quality, patient-centered care.

Ideal candidates have a CAPTE-accredited PT degree, PA license or eligibility, and strong evaluation and documentation skills. Experience in skilled nursing or geriatrics is a plus; PRN positions welcome both experienced therapists

Qualifications

• Graduate of a CAPTE-accredited Physical Therapy program.
• PA Physical Therapist license or eligibility to practice (PT Compact).
• Strong clinical evaluation, treatment planning, and patient care skills.
• Excellent communication, interpersonal, and documentation abilities.
• Ability to work collaboratively with an interdisciplinary rehabilitation team.
• Commitment to delivering individualized, high-quality rehabilitation services.
• Experience in skilled nursing, geriatrics, or post-acute rehabilitation is a plus but not required.

Responsibilities

• Evaluate and treat residents with orthopedic, neurological, and age-related conditions to improve mobility and independence.
• Develop and implement individualized, evidence-based physical therapy treatment plans.
• Provide skilled interventions including therapeutic exercise, gait training, balance activities, transfers.
• Monitor progress, reassess outcomes, and modify treatment plans accordingly.
• Educate residents, families, and caregivers on mobility, fall prevention, and home exercise programs.
• Collaborate with PTAs, OTs, SLPs, nursing staff, and other team members.
• Provide supervision to PT Assistants as required by Pennsylvania practice rules.
• Maintain accurate evaluations, progress notes, and discharge summaries per standards.
• Participate in interdisciplinary care planning and discharge planning.

What You'll Do

As a PRN Physical Therapist, you'll evaluate and treat residents with a variety of orthopedic, neurological, and age-related conditions, helping them improve mobility, restore function, and achieve their rehabilitation goals.

Your responsibilities will include:

• Perform comprehensive physical therapy evaluations to assess mobility, strength, balance, functional limitations, and rehabilitation needs.
• Develop and implement individualized, evidence-based physical therapy treatment plans based on patient needs and functional goals.
• Provide skilled therapeutic interventions, including therapeutic exercise, gait training, balance activities, transfer training, and functional mobility interventions.
• Help residents improve strength, endurance, coordination, safety, and independence with daily mobility.
• Monitor patient progress, reassess functional outcomes, and modify treatment plans as clinically appropriate.
• Provide education to residents, families, and caregivers regarding mobility strategies, fall prevention, safety, and home exercise programs.
• Collaborate with Physical Therapist Assistants, Occupational Therapists, Speech-Language Pathologists, nursing staff, and other interdisciplinary team members.
• Provide appropriate direction and supervision to Physical Therapist Assistants in accordance with applicable Pennsylvania practice requirements.
• Maintain accurate and timely evaluations, progress notes, treatment documentation, and discharge summaries in accordance with professional and regulatory standards.
• Participate in interdisciplinary care planning and discharge recommendations to support safe transitions of care.
• Follow established safety protocols, facility policies, and applicable state and federal regulations.
• Contribute to a positive, supportive, patient-centered rehabilitation environment.

Who We're Looking For
