Responsibilities:

• Perform evaluations in the home and develop personalized care plans.
• Assist patients with mobility and movement to regain independence.
• Create detailed treatment and care plans for each patient.
• Complete all documentation in accordance with policy and procedures.
• Communicate effectively via phone, email, and meetings with healthcare professionals, understanding hospice rules and regulations.
• Collaborate with team members to provide optimal care and services.
• Learn electronic medical systems such as OASIS and MatrixCare.

Qualifications:

• Graduated from an accredited Physical Therapy program.
• Licensed as a Physical Therapist in the applicable state.
• Knowledge of home health care; supervisory experience is a plus.
• Reliable vehicle, valid driver’s license, and current automobile insurance.
